Thrivership Care Plan

To ensure that you receive the highest quality care going forward, you will want to gather the following records that detail the care you have already received, and keep them together in a folder or binder:

TREATMENT TEAM

  • Write down the name, address and phone numbers (better yet, just grab their business cards!) of your medical oncologist, radiation oncologist, surgeon, plastic surgeon, internist or family medicine doctor, gynecologist, and any other provider such as a physical therapist, nutritionist, psychologist, or Chinese medicine practitioner

IMAGING REPORTS  (also ask the imaging center for a CD with the images)

  • Mammograms
  • Ultrasounds
  • Breast MRIs
  • Brain MRIs
  • CT or CAT scans
  • PET scans
  • Bone scans
  • Bone density scans
  • Chest x-ray
  • Liver ultrasound

PATHOLOGY AND LAB REPORTS

  • Your original biopsy reports
  • Your surgical pathology reports (from the lumpectomy, mastectomy and/or other operations)
  • Tumor markers (CEA, CA 27.29, CA 15-3)
  • Genetic mutation test results (i.e., the genetic test for BRCA, CHEK-2, etc.)
  • Special test results such as Oncotype Dx, MammaPrint, or Breast Cancer Index

TREATMENT SUMMARY REPORTS

  • Operative Notes
  • Chemotherapy summary of drugs received, doses, number of cycles
  • Hormonal therapy (like Tamoxifen or Arimidex)
  • Targeted drugs (like Herceptin)
  • Clinical trial identifying number and title (if applicable)
  • Radiation type and dose and fractions received

Customize Your Plan

Together with your doctors, make sure you create a detailed plan for ongoing care, such as:

  • Follow-up schedules for visits to each doctor
  • Testing schedules to screen for recurrence: imaging and blood work
  • Management of treatment-related side effects and/or complications
  • Management of other health problems, not cancer-related
  • Knowledge is Power: a list of signs and symptoms to report to your doctor
